WhatsApp appointment reminders for clinics: the sequence that cuts no-shows, what needs consent, and what must never be automated
Confirm with a message that asks for a YES, remind the day before and two hours ahead with the same question, treat silence as a slot to re-offer. Which messages are utility and which are marketing, and the clinical line an assistant never crosses.
A clinic cuts no-shows by confirming every appointment on WhatsApp with a message that asks for a YES, reminding the day before and two hours ahead with the same question, and treating silence as a slot to re-offer. Confirmations and reminders are utility templates about the patient's own booking, so they need no marketing consent; a recall for a check-up is marketing and does. Nothing clinical goes in an automated message, ever.

Why no-shows are a messaging problem
A patient who books on Tuesday for the following Thursday has, by Thursday, a different week than the one they planned. Most no-shows are not decisions; they are forgetting, or a clash nobody asked about in time. A reminder that says "you have an appointment tomorrow" is read and forgotten. A reminder that asks "reply YES if you are coming, or RESCHEDULE" gets an answer — and an answer of silence, by the evening before, is a slot the desk can re-offer.
The sequence
| When | Message | Category | Why |
|---|---|---|---|
| Enquiry arrives | Coordinator's name, which doctor or treatment, which days suit, then the next open slots | Free-form, inside the window | The first minute; the slot list is the next message |
| Slot booked | Doctor, date, time, floor, what to bring — "reply YES to confirm, or RESCHEDULE" | Utility | The YES is the point |
| Evening before | Reminder, same question | Utility | A no-show found now is a slot re-offered |
| Two hours before | Short reminder with the floor and the time | Utility | The one people actually act on |
| Report ready | "Your report is ready — collect it, or reply REPORT" | Utility | Never the result itself |
| Balance due | Amount, treatment, date, payment link | Utility | One reminder |
| Recall due | "It has been six months since your cleaning — want to book?" | Marketing | Consent, STOP, once per interval |
What must never be automated
- Anything clinical. A symptom, a dose, a result, "is this normal" — a rule hands the thread to a person before the assistant replies. The assistant books; it does not advise.
- The result itself. "Your report is ready" goes out. The report is sent only after the patient replies asking for it, in a thread they opened.
- Anything a third party could read as a condition. A reminder names the appointment ("review with Dr Nair"), not the diagnosis. The phone may be on a kitchen table.
Consent, and which messages need it
A confirmation, a reminder, a report-ready notice and a balance are all about a booking the patient made; under Meta's categories they are utility, and the booking is the consent. A recall — "it has been six months" — is Meta's marketing category whatever it feels like to the clinic, and it needs the patient's opt-in, a STOP line, and restraint: once per interval, not monthly. The consent guide has the rules for India.
Estimates and payments
After a consultation, when the patient asks, the estimate goes out as the price list's figure for that treatment with the number of visits and a validity date — never a number a model worked out. A balance after a visit is requested in the chat at the list amount, UPI or card inside WhatsApp where the clinic's number has a payment configuration, a Razorpay link otherwise. The payments guide covers the setup.
The templates
Seven WhatsApp templates for clinics — enquiry received, appointment confirmed, reminder, estimate, report ready, balance due and a check-up recall — each written to pass Meta's review and to say nothing about a condition. The clinics page shows the sequence running.
How much do WhatsApp reminders reduce no-shows?
Clinics that switch from a plain reminder to one that asks for a YES typically see no-shows fall by a third to a half, mostly because the ones who were not coming say so the day before and the slot is refilled. Your own number comes from your own desk; the mechanism is the question, not the channel.
Can I send a patient their test result on WhatsApp?
Only after they ask, in a thread they opened, and only as a document — never the result in the message body. The automated message says the report is ready and how to get it.
Is a check-up reminder a marketing message?
Under Meta's categories, yes. It needs the patient's opt-in and a STOP line, and it is sent once per recall interval. A reminder for an appointment the patient already booked is utility and does not.
